Please Note - Maximum 1 PHN session per class. If more than one grade 7 or 8 Growth & Development session
is being requested per school, please cluster these sessions consecutively on the request form.
All sessions must be booked by March 28, 2008.

Role of the Classroom Teacher:
- Complete parent information letter process
- Must be present during all PHN classroom sessions
- Provide class-set copies of Board-specific Student Worksheets at the beginning of the requested PHN session
