List of Special Supports Items that may be approved 

How to Apply

Special Supports Program does not fund equipment, devices or services that have already been ordered or purchased.
Do not order or purchase equipment prior to receiving funding approval. 

Applicants who reside in homes for special care, hospital or in a chronic or long-term care facility are not eligible for Special Supports.

• Ontario Works or Ontario Disability Support Program recipient
  Discuss your needs with your case manager. Take your note from the doctor or registered health professional to your case manager along with any cost quotations.
   
• Low income families or individuals
  
  A Request for Special Support Funding Application Form (pdf) must be completed to determine eligibility which includes looking at the applicant's: 
• income - Canada Pension Plan (CPP), Old age Security (OAS) other earnings, pensions
• assets - cash, bank accounts,  Registered Retirement Saving Plans (RRSP), stocks, bonds, secondary property, etc.
• allowable living expenses (rent/mortgage, utilities)
• cost of the special support item

Note:  Sponsored immigrants or those who have refugee status are not eligible for assistance through the Special Supports program.

A friend, family member or advocate working with the applicant may assist in the application process.  Some professional assessments (with the exception of dental services) are required along with the completed and signed application form.

List of required supporting documents:

• last year’s Notice of Assessment from the Canada Revenue Agency
• copies of pay stubs (if applicable)
• a letter of assessment from a healthcare professional
• a quote from a preferred vendor
