2003 Winner - Shalom Village Nursing Home

The Hamilton Long Term Care Project is a unique project in the province involving a partnership among all sectors of the Hamilton health care system to help improve the health and quality of life for the geriatric residents of Hamilton Long Term Care facilities. The partners include the Hamilton Emergency Services Network, local hospitals, home care, geriatric outreach teams and 21 public and private sector nursing homes. The project staff of 3 Nurse Practitioners (Dilys Haughton, Dave Far and Jane Jennings) provides cost effective, efficient and responsive primary health care services which are accessible to all 2,000 residents in Hamilton nursing homes. Data shows that nurses help to prevent unnecessary visits to the Emergency Room and hospital admission by rapidly responding to help manage acute illnesses and other health problems. The Nurse Practitioners also visit residents in hospital to help facilitate discharge and co-ordinate care back in the resident’s home / LTC facility. The leadership of the Nurse Practitioners in providing education, support and skill development for nursing home staff helps them provide more specialized care for people with complex health needs.
