Maintenance Request Form

This form is for residents of CityHousing Hamilton (CHH) to submit a maintenance request. Requests are monitored Monday to Friday between the hours of 8:30 am to 4:30 pm, excluding holidays. CHH will review your request by the end of the next business day. If this matter requires immediate attention, please call 905-523-8496. Please note staff will not enter if dogs are in unit without resident.

Any personal information collected by CityHousing Hamilton (CHH) will be used to create maintenance work orders. By providing your email address, you are consenting to receiving emails from the City of Hamilton and/or their agents/contractors for maintenance requests.. Information collected for this initiative may be stored on servers located in Canada and the United States and may be subject to Canadian and/or American laws.
