High School Vaccine Clinics

Students born in 2001, 2002 or 2003 who are missing mandatory school vaccines will receive a letter in the mail from Hamilton Public Health asking them to provide updated vaccine information to avoid possible suspension from school.  You can update your vaccine information with Public Health by:

An insert will be provided with this letter that offers the following high school clinics to these students.  Please note that your child will only be able to attend the clinic at their own high school.  If you do not want your child immunized at school, please fill out the opt-out form attached to the insert.  Send the completed opt-out form to Hamilton Public Health at least two days before the clinic in one of the following ways:

  • Phone: 905-546-2424 x7556 (to complete the form verbally with a nurse)

If we do not receive an opt-out form, high school students may provide consent for themselves and will be vaccinated at school.

We will be offering catch-up vaccines to students who require further protection from the following diseases: tetanus, diphtheria, pertussis, polio, measles, mumps, rubella, meningitis. The student will only be vaccinated if he/she is overdue for a vaccine.
