Accessibility Awards Nomination Form

Hamilton City Council has created an Accessibility Award Program for individuals, businesses, and community organizations and groups in Hamilton that have made a significant contribution, beyond legislative requirements, towards improving access for persons with disabilities. This advocacy can take many forms, including but not limited to through education and awareness, activism, community building, and improvements to properties or built environment.

Include up to two (2) testimonials supporting how the nominee has worked with others to create change. You may also include volunteer experience, photos, newspaper clippings or other supporting documents to help us understand what makes them Accessibility Trailblazers.

The City of Hamilton collects information under authority of Section 227 of the Municipal Act, 2001. Any personal information collected by the “2026 ACPD Accessibility Awards Nominee Form” will be used by City of Hamilton staff and members of the Accessibility Committee for Persons with Disabilities (ACPD) for the purposes of evaluating applicants' eligibility for the ACPD Accessibility Awards. By providing your contact information, you are consenting to be contacted from the City of Hamilton and/or their agents/contractors for the purpose of providing notifications and updates related to the 2026 Accessibility Awards.
