Home Management Program
Home Management works with individuals and families in their homes. It is a short-term life skills program that works with people who have the capacity and ability to perform the tasks independently.

Home Management uses practical instruction and coaching to meet the unique needs of each client to develop skills, such as:
Who can apply
- Low income families or individuals
- Ontario Works (OW) or Ontario Disability Support Program (ODSP) recipients
- Must live in the City of Hamilton
Making Referrals
Referrals are accepted from community agencies or individuals. To make a referral you will need to:
- Complete the Home Management Referral Form
- Download your completed referral form and attach in an email: [email protected]
One of our staff members will contact you to confirm we have received your referral.
You may also call our general line at 905-546-2424 ext. 4804 to have your application taken over the phone during business hours.
