Extensively Drug-Resistant Shigella Cases with Possible Sexual Transmission
Background
Hamilton Public Health Services (PHS) is notifying health care providers of the emergence of extensively drug-resistant (XDR) Shigella sonnei cases in Ontario. XDR Shigella are strains that are resistant to all commonly recommended antimicrobials. Sexual contact between men who have sex with men (MSM) has been identified as the predominant route of transmission.
To date Ontario has 10 confirmed cases of XDR Shigella sonnei in males with onset between March 29, 2022 and January 31, 2023. Four of these cases travelled abroad during their incubation period. Since 2021 several cases have been reported in several countries, including the US & Canada.

Reporting Requirements
Shigellosis is reportable to the Medical Officer of Health under the Health Protection and Promotion Act, R.S.O. 1990, c. H.7.
